THOROUGHBRED RACING ASSOCIATION OF OKLAHOMA BENEVOLENCE GUIDELINES
These guidelines have been established to outline the benevolence benefits funded by the TRUST division of the Thoroughbred Racing Association of Oklahoma. This guideline will briefly explain the outline of benefits and eligibility for the program. While this guideline is as accurate as possible, this document is not the final disposition.
FACTS:
This is NOT an insurance program, it is a Owner funded assistance program
This program is NOT intended as a SUBSTITUTE for private insurance
All TRAO members are urged to purchase insurance from a provider in case of a catastrophic illness
The benevolent Trust Committee has the FINAL determination of ALL claims
ELIGIBILITY:
TRAO members (Owners, Trainers, Asst. Trainers & licensed FULL-TIME employees for the current meet. (Full-Time employees must be employed for (30) days. Proof of employment must be presented (OHRC licenses and Payroll verification)
Eligibility starts once a member has (3) starts at the current meet
Owner, Trainer & Asst. Trainers dependents under the age of (18) are eligible
BENEFITS
MEDICAL: $2,500 PER CALENDAR YEAR (Includes RX)
DENTAL: $250 PER CALENDAR YEAR
VISION: $200 PER CALENDAR YEAR
DEATH: APPROVED ON AN INDIVIDUAL CASE BASIS
EMERGENCY: APPROVED ON AN INDIVIDUAL CASE BASIS
(No family shall receive over $4,000 per calendar year)
All appointments must be made by the Office Manager in the TRAO office. If you use your private physician, pre-approval must be obtained from the Office Manager at the TRAO office.
THE FOLLOWING PROCEDURES ARE NOT COVERED:
Any illness/injury derived from an employment situation (Workman’s Comp, etc.)
Any drug/alcohol substance abuse scenario’s
Cosmetic surgery
Mental/Nervous illness
No benefits after (15) days upon closure of the live meeting
No benefits if filed after (60) days of service
